TRAVEL ASSISTED CARE SERVICES
The Caregiver will: – Meet patient at their home, residence or facility and prepares them for the trip. – Assist patient to and from the vehicle. – Accompany and help the patient into the desired location. – Provide care to the patient during the visit. – Accompany the patient back to their home, residence or facility with stops for picking up prescription or grocery. – Ensure the patient is settled in their home before departing.
